Now that Hilary Duff is coming back to the music scene, nostalgia is motivating droves of fans back into her corner. But with laser focus on the "Lizzie McGuire" star, people are starting to notice a drastic change in her appearance. Duff has undoubtedly gone through a stunning transformation since her fame in the early aughts. However, netizens have decidedly called her out for her perceived overuse of cosmetic alterations. Duff's aesthetic adjustments have been the topic of headlines for over a decade — namely, her veneers. But over time, the narrative has changed from her smile to her frown (lines) and beyond.
An April 2025 selfie on Instagram sparked vitriol about her youthfully plump visage. "Why did she do Botox, I swear to god everyone else is the same," one user commented. "[It's] cheek filler not botox," another surmised under the first comment. The post ignited such a frenzy of speculation about Duff's use of plastic surgery that her husband, Matthew Koma, stepped in to tell off the initial Instagram user who sparked the discussion.
Unfortunately, the plastic surgery rumors have migrated from Duff's Instagram comment section and spread across the internet. After photos of the bare-faced "What Dreams Are Made Of" hitmaker on an outing in Los Angeles were published in May 2025, a Reddit thread came after her appearance, with the originator calling out Duff for her supposed "Madonna-esque cheek filler" — referring to the celeb's unrecognizable face transformation.

Hilary Duff hasn't directly confirmed whether she has gotten any work done. She once alluded to receiving injections in a 2022 interview with Women's Health, but her overall message is more profound. "We bust our ass to get our bodies in shape and to look the best we can. We get facials and Botox and our hair done and highlights and brows and lash lifts and all this s***," said Duff. "But I want to work on the inside. That's the most important part of the system." The "How I Met Your Father" actor consistently goes to therapy, but also grounds herself by focusing on what's in front of her: her home, her children, her husband. " ... that's been the easiest way to become a full person," she told Harper's Bazaar in 2022.
Being a mom has solidified her body positivity journey, but before that, Duff struggled with her image. In her interview with Women's Health, Duff laid bare her struggles with disordered eating as a teen. "It was horrifying," she said of the measures she would take to feel Hollywood thin. In her adult years, she has been more focused on being strong and associating herself with people who make her feel good. Ultimately, that likely means her social media haters are being shunned.
